Central Serous Chorioretinopathy

Central serous chorioretinopathy (CSC) is a usually sudden-onset visual disturbance characterized by a build-up of fluid beneath the retina. It is most often seen in young and middle-aged men and usually affects a single eye.

Quick Answer

Central serous chorioretinopathy occurs when fluid leaking from the choroid collects beneath the macula, causing blurring and distortion in the central vision; in most cases it tends to resolve on its own within a few months.

Central serous chorioretinopathy (CSC) is a usually sudden-onset visual disturbance characterized by fluid accumulating beneath the retina. Fluid coming from the vascular tissue known as the choroid, located beneath the retinal layer, collects under the macula, the central area of the retina. This leads to blurring, distortion of shapes and image shifts in the central vision.

The condition is frequently seen in young and middle-aged men. It usually affects a single eye, but in some cases both eyes may be involved.

Frequently Asked Questions (FAQ)

Why Does Central Serous Chorioretinopathy Occur?

Although the exact cause of CSC is not fully known, certain conditions increase the risk of the disease developing:

  • Stress: The most common trigger. It can affect vascular permeability by increasing cortisol levels.
  • Cortisone (steroid) use: Cortisone-containing medications taken orally, nasally or through the skin can increase the risk.
  • High blood pressure and sleep disorders
  • Pregnancy
  • Type A personality structure (perfectionist, stressed individuals)
  • Genetic predisposition

What Are the Symptoms?

Central serous chorioretinopathy usually develops suddenly and causes the following symptoms:

  • Blurring of vision (especially in the center)
  • Straight lines appearing curved or wavy
  • Colors being perceived as faded
  • A dark spot in the middle of the visual field
  • Objects appearing smaller (micropsia) or larger (macropsia) than they are
  • Light sensitivity

These symptoms are usually painless and are seen in a single eye in most patients.

How Is It Diagnosed?

In addition to the eye examination performed to diagnose CSC, some advanced imaging methods are used:

  • OCT (Optical Coherence Tomography): The fluid accumulation beneath the retina is clearly imaged.
  • FFA (Fundus Fluorescein Angiography): The location and type of the fluid leakage are determined.
  • ICG (Indocyanine Green Angiography): The choroidal vessels are examined in detail.

These imaging studies are important for treatment planning according to the stage of the disease and whether it recurs.

Central Serous Chorioretinopathy Treatment

In a large proportion of patients, CSC can resolve on its own within a few months. During this process:

  • Stress management
  • Discontinuing cortisone-containing medications (under a doctor’s supervision)
  • Close follow-up

are recommended.

However, in some patients the fluid accumulation lasts a long time or recurs. In these cases treatment is needed:

Laser Photocoagulation

The leakage area is identified and closed with a low-dose laser. It can be applied in selected cases.

PDT (Photodynamic Therapy)

It is used in more resistant or chronic cases. Targeted vessels are closed with special light-activated medications.

Anti-VEGF Injections

This is an intraocular injection treatment aimed at reducing fluid accumulation. It may not be suitable for every patient.

Does CSC Cause Permanent Vision Loss?

Usually not. In most patients, vision returns to normal over time. However:

  • If the condition recurs frequently
  • If treatment is delayed
  • If it becomes chronic

permanent damage may occur in the macula, and this may lead to a permanent decrease in vision. For this reason, regular follow-up is important.

Who Is at Risk?

  • Men between the ages of 25 and 50
  • Those working under chronic stress
  • Those using cortisone medications
  • Those experiencing sleep disorders
  • Individuals who have previously had CSC

Frequently Asked Questions

Why is CSC usually seen in men?

CSC appears more frequently, particularly in men between the ages of 25 and 50. The exact reason is not fully known, but it is thought that hormonal and vascular sensitivity in men may trigger this picture. Differences in stress and cortisol levels may also play a role.

If CSC resolves on its own, is treatment still needed?

In most cases, yes. CSC usually regresses on its own within a few months. However, close follow-up during this period is important. To guard against the risk of permanent damage to the central vision, your doctor determines the timing of any intervention by monitoring the course of the fluid accumulation.

Does this condition recur?

Yes, it may recur. In some patients who have had CSC, fluid accumulation may develop again over the years in the same eye or in the other eye. The risk of recurrence increases especially in individuals under chronic stress.

Does CSC cause vision loss?

It usually follows a course with temporary visual disturbance. However, if the fluid remains beneath the retina for a long time or if the condition recurs frequently, permanent deterioration may occur in the central vision. This may lead to a permanent decrease in vision.

Can I continue working with this condition?

Usually yes. However, blurred vision and image distortion may cause temporary performance problems, especially in those who work in front of a screen. It is helpful to take measures that reduce eye strain and to take rest breaks.

Is CSC related to stress? Does it occur only because of stress?

Stress is one of the biggest triggers. High stress can affect vascular permeability by increasing cortisol levels in the body. However, cortisone medications, genetic factors and sleep disorders can also influence the condition. So it is not stress alone, but several factors playing a role together.

I use cortisone; can CSC develop?

Yes. Medications containing cortisone (including tablets, creams and nasal sprays) can increase the risk of CSC. For this reason, patients using cortisone chronically in particular are advised to take their eye complaints seriously and to have a retinal examination.

Will I fully recover after treatment?

Most patients regain normal vision after treatment. However, in some people permanent traces such as mild fluctuations in visual quality or a decrease in color sensitivity may remain. This is generally seen in cases of late diagnosis or those that have become chronic.
